Instructions for completing and submitting the Moorage License Agreement

To complete the form below:

1.       Download or save document in Adobe, complete, and submit electronically using the “Submit to Port” button at the bottom of the first page. (preferred)

2.       Complete online, print, and submit paper document in person.

Send in a copy of your Certificate of Insurance naming the Port of Newport as an additional insured. If you have questions, please email info@portofnewport.com or call 541-265-7758. Please note if the form is opened in a web browser, the submit button will not work. The form has to be opened in Adobe.
